Forms Available in the Template Library

A description of the default eForms available for your use in the Forms Template Library button on your Admin > Forms dashboard.


 

PeerAssist offers a full library of standard form templates for your use. Each template can be copied and edited to suit your needs. Change the title, section headers and content. These templates are a great starting point for your team.

OSHA and Covid-19 Compliance

  • Covid Health Assessment
    • This form uses Yes/No/NA checkboxes to determine if your employees are eligible to work on the jobsite or in the office. This form offers suggestions to protect yourself and others along side a series of Covid-19 related health questions.
  • Employee Health Self-Certification
    • This form uses Yes/No/NA checkboxes and a signature section to help your employee certify their compliance with local health regulations and eligibility to work on the jobsite. 

 

 

Daily Reports

  • Daily Construction Report
    • Use this form to report daily construction progress including physical conditions, general information regarding task codes and work completed, equipment usage and  manpower.
  • Daily Job Report
    • This form covers on site manpower for a range of classes, daily work performed and problems or delays encountered on the jobsite.

 

 

Inspections

  • Daily Site Inspection
    • Use this form to help your foreman or superintendent acknowledge compliance with job site safety regulations for signage, housekeeping, PPE, fall protection, tools/equipment and Covid-19 safety guidelines.
  • Exterior Scaffold Inspection
    • This form uses Not Applicable, Satisfactory and Correction Needed (NA/S/C) checkboxes to help ensure the safety of all scaffold components and planking conditions. A signature may be captured by the foreman or super in charge when all aspects of setup are complete.
  • Ladder Inspection
    • This form uses Not Applicable, Satisfactory and Correction Needed (NA/S/C) checkboxes to help ensure the safety of ladder conditions. A signature may be captured by the foreman or super in charge when a full ladder inspection is complete.
  • OSHA Inspection
  • Scissor Lift Inspection
    • Use this form to verify the scissor lift jobsite working conditions, functions and controls, platform assembly, scissor arms, chasse, power, hydraulics, manuals, usage and capture additional notes.
  • Vehicle Inspection
    • Use this from to verify vehicle registration, insurance, interior and exterior conditions, maintenance and required safety equipment.

 

 

Accidents, Damage and Investigations

  • Incident Investigation
    • This form uses the Comment section type to help you capture details of any personal injuries or property damage incidents, causes and cause analysis. Use this detailed form as supportive documentation for police or company reports.
  • Equipment Damage Report
    • This from uses the Item Selection section type to allow the user to select items from your master material, equipment and rentals list, as well as Y/N/NA checkboxes to report jobsite equipment damages.
  • Equipment Usage Report
    • This from uses the Item Selection section type to allow the user to select items from your master material, equipment and rentals list, as well as Y/N/NA checkboxes to report jobsite equipment usage and inspection verification.

 

 

Planning

  • Tailgate Meeting with Attendance
  • Task Planning
    • Use this form to fill in general details about planned tasks like start/end times, housekeeping, material delivery and storage, as well as work task requirements and conditions. Allow assigned employees to sign in acknowledgement.

 

 

Quality Control

  • Drywall QC
    • This form uses Y/N/NA checkboxes with a comment section to report on quality control for drywall subcontractors. Questions include onsite material availability, frame-outs, caulking requirements, fasteners and joints finishes.
  • Painting QC
    • This form uses NA/S/C fields to monitor the paint quality control on the jobsite. Pre-task items include, run drips inspection, window mullions and door over-spray or splatters. Post-task items include wall conditions, sanding, tie-in joints, existing patches and priming.

 

 

Tools and Materials

  • Tool Inventory
    • This from uses the Item Selection section type to allow the user to select items from your master material, equipment and rentals list or to enter custom items for your tool inventory checklist.
  • Field Material Request
    • This form is designed to be used in conjunction with our Purchasing > Field Material Request program. It uses the Item Selection section type to allow you to select additional items from your master material, equipment and rentals list required for the project. Prices are hidden from the field user's view and the form can be sent in to the office for approval.
